Online Request Forms
There are two forms to download which are the “Admission Form” and “Urine Test Form”. Please read carefully the following steps to download the forms and join STIKOM LSPR – Jakarta.
Step-1: Download the Admission Form:
- Click one of the following forms to download:
- Formulir_Registrasi_LSPR.doc (MS-Word) or
- Formulir_Registrasi_LSPR.pdf (Adobe PDF) - Print the “Admission Form”
- Fill up the “Admission form” completely and read the steps carefully
Step-2: Download the Urine Test Form:
- Click one one of the following forms to download:
- Surat_Test_Urine_LSPR.doc (MS-Word) or
- Surat_Test_Urine_LSPR.pdf (Adobe PDF) - Print the “Urine Test Form”
- Fill up the “Urine Test Form” completely and read the steps carefully
- Please go to Jakarta Hospital (RS. Jakarta), Jl. Jend. Sudirman kav. 49. Jakarta, to do the urine test.
- You will receive the results in a closed envelope. Please do not see the result. THE DOCUMENT WILL NOT BE ACCEPTED IF THE ENVELOPE IS DAMAGED/TORN. You will know the results when you return all requirements & forms to Marketing Department.
Step-3: Procedure of Returning The Forms
The next step is returning the forms to the marketing office of STIKOM The London School of Public Relations – Jakarta to receive the official number of the Admission Form and to follow the Entry Test:
- Check if the “Admission Form” has been completed.
- Bring all requirements that was stated in the Admission Form (photos, copy of ID, copy of high school diploma, results of the urine test) to Marketing Department, Campus A, Wisma Dharmala Sakti – annexe building, 6th floor, Jl. Jend. Sudirman kav. 32. Jakarta, at least one day before the examination day. (Check all requirements and schedule of entry test here or click Registration Guides link on the left)
- You will be informed to complete the procedure of payment to join the entry test.
- After you complete the payment, you will receive the official examination card.
- Please keep the official examination card and re-check your entry test schedule.
